It comes from many similar conversations, rather than any one patient\u2019s story.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">If I put that repeated experience into words, it often sounds something like this:<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><em data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"em\">\u201cI wake up tired, even when I have slept for long enough. Coffee gets me through the morning, but by the afternoon I am dragging myself through the rest of the day.<\/em><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><em data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"em\">Then, when evening comes, I suddenly feel more alert. My body is exhausted, but my mind starts going again. I lie down wanting desperately to sleep, yet I cannot properly switch off. If I do fall asleep, I often wake during the night, and the next morning it starts all over again.<\/em><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><em data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"em\">But I don\u2019t think I\u2019m stressed. This is simply what my life is normally like.\u201d<\/em><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">That final sentence is often an important clue.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Exhaustion and nervous-system activation can exist together<\/strong><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">We tend to imagine that an exhausted person should be able to fall asleep easily and wake feeling restored.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">But the body can be running low on energy while the nervous system continues behaving as though it needs to remain alert.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">This may feel like:<\/p><ul class=\"ak-ul\" data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"bulletList\" data-prosemirror-node-block=\"true\"><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Physical exhaustion with a mind that will not slow down<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Feeling flat during the day but getting a \u201csecond wind\u201d at night<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Falling asleep but waking repeatedly or too early<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Feeling tired yet restless<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Relying on caffeine to function, only to feel more tense or unsettled<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Sleeping for enough hours without feeling genuinely restored<\/p><\/li><\/ul><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Feeling wired and exhausted at the same time is not a contradiction. It is a recognisable pattern, and that in itself can provide useful clues about what the body is trying to manage.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Why simply sleeping more may not solve it<\/strong><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Sleep quantity is only one part of the picture. A number of things can contribute to fatigue:<\/p><ul class=\"ak-ul\" data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"bulletList\" data-prosemirror-node-block=\"true\"><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Poor-quality sleep<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Hormonal changes<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Fluctuations in blood sugar<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Prolonged physical or emotional strain<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Inflammation or pain<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Nutrient deficiencies<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Medication effects<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Stimulants such as caffeine<\/p><\/li><\/ul><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">This is why two people who both say, \u201cI am exhausted,\u201d may need completely different forms of support.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">One person may need us to look more closely at sleep quality. Another may be experiencing hormonal changes, unstable energy after meals, persistent inflammation or a nervous system that has become accustomed to remaining on guard.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">The word \u201cfatigue\u201d describes how someone feels. It does not tell us why they feel that way.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Four things you can notice this week<\/strong><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">You do not need to diagnose yourself. Simply observe, and if you are willing, try one small experiment.<\/p><ol class=\"ak-ol\" start=\"1\" data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"orderedList\" data-prosemirror-node-block=\"true\"><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">When does your energy change?<\/strong><br data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"hardBreak\" data-prosemirror-node-inline=\"true\" \/>Notice whether you wake tired, crash after meals, fade in the afternoon or become more alert at night.<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Are you sleepy, or exhausted but unable to settle?<\/strong><br data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"hardBreak\" data-prosemirror-node-inline=\"true\" \/>These are not always the same experience.<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">What happens before a difficult night?<\/strong><br data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"hardBreak\" data-prosemirror-node-inline=\"true\" \/>Consider late caffeine, alcohol, skipped meals, intense exercise, emotional pressure or a particularly demanding day.<\/p><\/li><li data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"listItem\" data-prosemirror-node-block=\"true\"><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Experiment with delaying your first coffee.<\/strong><br data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"hardBreak\" data-prosemirror-node-inline=\"true\" \/>If you normally reach for coffee immediately after waking, try waiting about 45 minutes for one week. Notice whether your energy feels steadier, whether you feel less restless or jittery, and whether you still need the same amount of caffeine later in the morning.<\/p><\/li><\/ol><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">These small details often tell us more than the word \u201ctired\u201d alone.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\"><a href=\"https:\/\/drdebbie.co.za\/drdebbie\/treatments\/acupuncture\/\"><strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">How I approach this pattern<\/strong><\/a><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">In consultation, I do not begin with, \u201cWhat can we give you for energy?\u201d<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">I begin with two different questions: <strong data-prosemirror-content-type=\"mark\" data-prosemirror-mark-name=\"strong\">Why is your energy low, and why is your system struggling to switch off?<\/strong><\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">I want to understand when this began, what changed before it started, how your energy moves through the day, what happens during the night and what other symptoms occur alongside it.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">We may need to consider sleep, hormones, digestion, blood-sugar regulation, inflammation, nutrient status, medication and the nervous system as parts of the same picture. Existing results may need to be reviewed, and sometimes further investigation or referral is appropriate.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">Only then can we decide whether the next step involves adjusting your current treatment, acupuncture, lifestyle changes or carefully selected nutritional, herbal or homeopathic support.<\/p><p data-prosemirror-content-type=\"node\" data-prosemirror-node-name=\"paragraph\" data-prosemirror-node-block=\"true\">I do not choose a supplement simply because someone feels tired.
